Treatment Agreement
Before your first treatment at Integrative Health Clinic, I need you to be awere of this Treatment Agreement. It explains what you can expect from Integrative Health Clinic, what I ask of you and how your personal and medical information is handled. Fully in line with Dutch healthcare regulations (WGBO, Wkkgz) and the relevant professional associations (NVVM, VBAG, RBCZ). By confirming your appointment, you confirm that you understand and agree to the terms below.

1. Consent to Treatment
The undersigned hereby gives consent for Integrative Health treatment at Integrative Health Clinic. This consent remains valid until withdrawn, or until treatment is terminated, with due observance of the prescribed retention period.
By confirming your appointment, you also give the Health Practitioner permission to send invoices and other correspondence by email.
2. Legal Framework and Client File
The Health Practitioner acts in accordance with legal regulations, including the Medical Treatment Agreement Act (WGBO) and the Quality, Complaints and Disputes in Care Act (Wkkgz).
The Health Practitioner keeps a client file. The client has the right to access their own file, which is retained for 20 years in accordance with the statutory retention period (see PR 520 – File Handling for RBCZ sectors).
3. Information, Consent and Shared Decision-Making
The Health Practitioner must provide information regarding the treatment at all stages of the process. The client must provide all relevant information during the intake (anamnesis) interview.
The Health Practitioner may not perform any Integrative Health treatments or procedures without the client’s consent. In addition to the practitioners duty to inform, emphasis is placed on joint decision-making between practitioner and client regarding the course of treatment.
4. Referral
The Health Practitioner must appropriately refer the client to a colleague therapist, practitioner or physician if Integrative Health treatment is not suitable and/or sufficient.
5. Termination of Treatment
Treatment may be terminated at any time by mutual consent. The client may terminate treatment unilaterally at any time. The Health Practitioner may only terminate treatment unilaterally by stating reasons and, if necessary, referring the client to another therapist, practitioner, doctor, or other healthcare provider.
6. Privacy Statement Acknowledgement
The client confirms they have read and agree to the Privacy Statement published on this website.
7. Terms and Conditions Acknowledgement
The client confirms they have read and agree to the General Terms and Conditions of Integrative Health Clinic, published on this website.
8. Fees and Payment
The client agrees to the consultation rates listed on this website. Payment is made via payment request, credit card, debit card or cash and fulfilled immediately after the consultation.
9. Cancellation Policy
The client must cancel an appointment at least 48 hours in advance. Appointments cancelled within 48 hours will be charged in full.
10. Complaints
If you have a complaint about your treatment, I will first attempt to resolve it together through direct discussion and/or treatment.

For complaint about your treatment, the client may contact the VBAG.
11. Liability for Withheld Medical Information
The client is fully responsible and liable for any adverse consequences resulting from withholding information contained in the medical file held by their general practitioner.
